I had no idea what that strange bit Bono was saying was either. It seemed really odd and definitely some sort of inside joke.
However, over the weekend I was in a store and heard the old song "Ballroom Blitz" (by a band called "Sweet") come on the radio. At the beginning of the song, there it was, "Are you ready, Steve, Ah. Andy? Yeah! Mick? OK. Alright, fellas, let's go!".
This is an older song (at least 20+ years old) but it is clearly where Bono got it from. I have no idea why he chose to add it to the Milan performance of Vertigo. (Perhaps just to add fodder for the "trainspotters" as he calls them.) The lyrics to the entire song are below...
