Crate training really does make a difference, as well as giving them lots of opportunities to go outside and praising them when they go in the right place, no punishments for accidents inside (pick them up and take outside if you catch them in process). Another thing to keep in mind - in general, puppies pretty much cannot be fully trained before 16 weeks old. Their bladder/urinary muscles are not developed enough to be able to hold against much pressure.
