This often happens with the elderly - they deteriorate rapidly following an accident or an injury like a broken hip.

My grandfather was in pretty good shape for his age (83), with some Alzheimer's and early memory loss, but he could still function largely independently. Then he had a heart attack and he never really recovered from it mentally. He is alright walking around, he's actually pretty spry, but his memory is awful, he has delusions and had to be placed in a nursing home because he could no longer take care of himself.

It can happen almost overnight so that it's shocking, but it's not uncommon at all.
