It's an actual photo from Vanity Fair, that's Graydon Carter (editor). It was their Christmas card that year. There's video of the photo shoot too. I have it, it might be on you tube, I'll check. (Annie Liebovitz took the photos of them separate I believe, then VF PSed them together.)

Edit: I couldn't find it on YT, but I'll upload it.
