Not quite on that level, but when I was in high school, there was a bit of a stuff-up with such a test. They were testing a couple of sirens, so the teachers were told not to evacuate their classes. So there were a number of short tests as expected and we kept working ... then they ran it again and didn't stop. After about three minutes of this, the teacher of my class decides maybe we should go out to the oval after all. Turned out certain senior school administrators were out there, very upset that no classes had evacuated!
