I can only assume it refers to his repiratory problems, resultant from smoking and such, that caused such issues with his voice back then, and I'm sure, had side effects for his health as well, it's possible they're over-dramatizing a touch, I'm not really sure. Don't quote me.

Hmmm...I'm not sure, and I've read "U2 at the End of the World," which is damn comprehensive for the band during the 90's. As Brightest said, I'm assuming it was the problems he had with his voice from overuse and smoking. Let me know if you hear more...
