Use the link to the right of the video, and just simply copy and paste that link in your post.
I think when people try to use the link in the URL bar or try to paste it in the advanced URL bar box in your reply it screws everything up.

Which link do I use? The URL just pasted into my post, or the Embeded text just pasted into my post?

I used to sometimes put the YouTube URL into an IMG quote tag 
Always broke the thread, but luckily I'd catch it and edit it.
